CACC Event Incident Report

Use this form to report a collision or injury occurring at any CACC-sanctioned event. In the event of injury or fatality, obtain as many written eyewitness reports as possible, including name, address and contact numbers and attach reports to this form. Combine and attach additional information as needed.

The CACC also provides Track Report and Steward’s Incident Report forms on the CACC website forms page to document incidents that occur during a CACC-sanctioned event. The Track Report is a handwritten document that can be used by course workers at any event and covers incidents not resulting in injury or collision. The Steward’s Event Report is submitted online and incorporates both minor and significant events involving collision or injury.

Your submitted report will be kept on file at the CACC. A copy of the report will be sent by email to the CACC Steward, the appropriate CACC Discipline Director and to the email address that you provide below. If an injury or fatality is indicated, information from this report will be forwarded automatically to CACC’s insurer.
